A copy of your form has been emailed to you. Please have all relevant medical records emailed to [email protected] at least 1 hour prior to the start of your appointment so the doctor can review them.

In a separate email, you should have received an invoice. The invoice must be paid prior to the start of your appointment. We have also sent you a google meeting invite to the teleconsult.
